Using the correct form to request authorization or to notify the correct parties of your planned activities will help insure a successful cruise. Please read these forms carefully and provide all the required information.

	Requesting the use of a research vessel: Ship Request Form (HTML) - an on-line request form for NSF-UNOLS Ship time Small Vessel Request (PDF) - a mail or fax back form to the local Port Captain

	Foreign Clearances: (forms are specific to each vessels) R/V Thompson UN Standard for cruises into waters of Foreign Nation (MSWORD) R/V Barnes UN Standard for cruises into waters of Foreign Nation (MSWORD)

	Science Party Authorization: Personal Data Forms (both sides) must be completed: Individual Science Party Member Personal Data Form - SIDE ONE - (PDF) Individual Science Party Member Personal Data Form - SIDE TWO - (PDF) Use this older form if instructed Memeber of Scientific Party Authorization (PDF) All Science Party member are required to have review these procedures Science Party Pre-Cruise Agreement (MSWORD) Science Party Vessel Familarization, Policy and Procedures Agreement (MSWORD) NOTE this form is for pre-cruise refernce only and will be filled out on the ship, not before the cruise
